General Information on Appointments

Which Appointment Should I Request?

Call and describe the main reason for the visit. Back pain, neck pain, sciatica, disc concerns, extremity symptoms or questions about spinal decompression generally begin with a chiropractic appointment. Medical weight-management or peptide-related questions begin with a coordinated telehealth consultation through a licensed medical provider. The team can explain the process, but the appropriate clinician makes the clinical decision.

What Happens After You Book

Once an appointment is confirmed, the practice emails the applicable intake forms. Completing them before the visit gives the provider time to review the history and helps the appointment focus on the current concern. Bring existing imaging and radiology reports when available; additional imaging is not automatically required for every patient.

For chiropractic appointments, the first visit may include history, orthopedic and neurological testing, movement assessment and review of relevant imaging. Treatment may begin at the first or a later visit depending on the findings. For wellness consultations, the licensed medical provider determines whether further records, testing or follow-up are required.

Payment and Insurance

Revive does not accept insurance and operates on a self-pay basis. Cash, Zelle and credit cards are accepted, and financing is available. Ask the practice for current fees and financing terms before beginning care.

Fees and Estimates

Revive is a self-pay practice. Before beginning care, you can ask about current visit fees, package terms, financing, cancellation rules and your right to request a Good Faith Estimate. Buying a package does not guarantee a result, and recommendations may change after re-evaluation.

Medical Emergencies

Revive does not provide emergency medical care through the website or contact form. Call 911 or go to the nearest emergency department for a medical emergency.
