Self-Pay and Payment Policy
Revive is a self-pay practice and does not bill insurance. Patients are responsible for charges for services they agree to receive. Accepted payment methods currently include cash, Zelle and major credit cards. Financing may be available subject to separate terms and approval.
Fees, Care Plans and Packages
Fees, package terms, included services, review points, expiration rules and refund conditions must be explained in writing before purchase. Clinical recommendations are based on an individual evaluation and may change.
Purchasing a care package does not guarantee a result or remove your right to ask about alternatives, fees or the reasons for a recommendation.
Appointments, Cancellations and No-Shows
Appointments are required. Revive requests at least 24 hours’ notice to cancel or reschedule.
Emergency or exceptional circumstances will be considered according to the written practice policy and applicable law.
Good Faith Estimate Notice for Uninsured or Self-Pay Patients
Your Right to an Estimate
If you do not have health insurance, or you choose not to use insurance for your care, federal law generally gives you the right to receive a Good Faith Estimate explaining the expected cost of scheduled items and services. You may also request an estimate before scheduling.
What to Expect
Revive will provide the estimate within the time required by applicable law based on when care is scheduled or requested. The estimate is based on information known at the time and may not include unexpected services reasonably required during care.
A Good Faith Estimate is not a contract and does not require you to obtain the listed services.
If Your Bill Is Substantially Higher
You may have the right to use the federal patient-provider dispute resolution process if a bill from a provider or facility is at least $400 more than its Good Faith Estimate. Keep a copy or photograph of your estimate and bill.

Telehealth and Coordinated Wellness Notice
How Wellness Services Are Coordinated
Revive may coordinate selected wellness consultations delivered by licensed medical doctors through telehealth. The medical provider – not the chiropractor, website or marketing content – performs the medical evaluation and decides whether laboratory testing, prescribing, monitoring or referral is appropriate.
Separate Clinical Responsibilities
Before receiving care, you should be told:
- The legal name and contact information of the prescribing or treating medical entity.
- The clinician’s professional license and role.
- The telehealth platform used for the consultation.
- Who maintains the medical record.
- Who bills for each service.
- Which privacy notice and patient policies apply.
No Guaranteed Eligibility or Result
Categories such as weight management, sexual health, systemic inflammation, pain, hormonal concerns, skin or hair rejuvenation and soft-tissue repair describe reasons you may ask for a medical consultation. They are not promised benefits or approved indications for a specific peptide or compounded product.
The licensed medical provider determines eligibility, alternatives, risks and monitoring.
Compounded Products
Compounded drugs are not FDA-approved. FDA does not verify their safety, effectiveness or quality before marketing. The prescribing clinician and dispensing pharmacy must provide product-specific information, informed consent and required disclosures.
Emergencies, Privacy and Technology
Telehealth is not appropriate for every condition or emergency. Call 911 for an emergency. Technology can fail, and electronic communications carry privacy risks. The treating medical provider’s telehealth consent and contingency process govern the medical visit.
